Using your advanced practice nursing skills, you will assess, coordinate care and provide education for a diverse patient population. This will involve collaborating and consulting with a multi-disciplinary clinical/professional team as well as patients and families to ensure safe and effective coordination of care. You will:
- Assess, coordinate, develop and implement individualized care plans
- Utilize evidence-based tools for risk stratification
- Perform utilization reviews, including authorization reviews
- Ensure delivery of concurrent and post-hospital care
- Initiate referrals, coordinate resource utilization, and evaluate post-acute care services
- Participate in performance improvement projects/processes
- Implement a comprehensive DC plan for high-risk patients
Salary Range: $63,800 – $127,000/Annually
We’re seeking a self-directed, assertive, creative problem solver with:
- CA LVN licensure
- At least three years of acute hospital experience required
- Two or more years of utilization management, case management or discharge planning experience in an HMO environment required
- Team leading or management experience
- Systems planning and care management background in a high-volume setting
- Proven resourcefulness with strong communication, diplomacy, organizational, prioritizing, and analytical skills
- Ability to work effectively and collaboratively with interdisciplinary teams
- Computer proficiency
- Understanding of the functions of a large university teaching hospital
- Ability to abstract and interpret medical information from patient records
- Familiarity with the laws, rules, and regulations regarding utilization review and discharge planning of government programs such Medicare, Medi-Cal, and CCS
- Ability to develop individualized case management plans that address physical, vocational, psychosocial, financial, and educational needs
