Make a difference in the community as a RN Case Manager with our Venice Family Clinic. With guidance and direction from the Director of Nursing and Director of Homeless Services, the Homeless Program RN Case Manager is responsible for providing compassionate and comprehensive case management services to individuals experiencing homelessness. This role involves conducting health assessments, developing personalized care and housing plans, and advocating for clients’ access to housing and healthcare resources. The RN Case Manager works collaboratively with community organizations and service providers to navigate complex systems, ensuring that clients receive the support they need to achieve and maintain stable housing and overall well-being. Through ongoing case management and advocacy, this position helps empower vulnerable populations to overcome barriers and improve their quality of life overcome barriers and improve their quality of life.
Salary range: $129,017.52 – $166,852.08 Annual
- BSN or MSN from an accredited school of nursing (ASN required for current UCLA Health RN staff).
- Current, active California Registered Nurse License.
- BLS certificate from American Heart Association or American Red Cross.
- Minimum of 2 years of relevant experience in case management, including 1 year of experience working with the unhoused or similar populations.
- Bilingual Spanish/English verbal language skills.
- Ability to develop care plans and assist clients with linkages to medical, psychiatric, social, educational, housing, food, and other necessary services.
- Ability to collaborate with clinicians and community health workers to assess patient needs and provide tailored enhanced case management addressing both clinical health needs and social determinants of health.
- Excellent verbal, written, and interpersonal communication skills.
- Demonstrated skill in delivery of patient care.
- Ability to move office materials up to 25 pounds.
- Valid CA Driver’s License, insurance, and personal transportation; including ability to drive mobile clinics as needed.
