Under the supervision of the Hematology/Oncology Community Practice Director of Nursing, the Community Practice Oncology Nurse Navigator (CPONN) is a professional Registered Nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help navigate the healthcare system. Using the nursing process, the CPONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The CPONN serves as an additional point of contact for patients, families, caregivers, and referring physicians to provide resources and assistance with accessing clinical and supportive care services offered within and outside of the assigned UCLA Hematology Oncology Community Practice Cancer Center.
Salary range: $68.79/hr – $98.22/hr
Required:
- Possession of an BSN or MSN degree
- Possession of current California RN license
- Possession of BLS certification from the American Red Cross or American Heart Association
- Possession of ONS/ONCC Chemotherapy/Immunotherapy certification
- Possession of OCN certification
- Demonstrate strong interpersonal and organizational skills to interact courteously and effectively develop and maintain cooperative working relationships with physicians, patients and their families, managers, and staff from various social, cultural, and economic backgrounds
- Ability to organize, prioritize, and follow through for multiple patient treatments on an ongoing basis
- Minimum of five years of acute care/oncology inpatient and/or outpatient experience
- Experience in accurately screening and triaging acute patients
- Knowledge to recognize abnormal laboratory values and report to Physicians
- Ability to establish and maintain cooperative working relationships with patients, peers, physicians, and administrators
- Ability to accept direction from leadership and act promptly
- Skill in listening perceptively, conveying awareness, and responding to patients and family/caregivers with health concerns using tact and discretion
- Ability to delegate tasks in a management style that is consistent with promoting leadership and teamwork.
- Ability to interact diplomatically and sympathetically with patients, their families, and the general public in a high-volume, clinical setting
- Skill in performing with frequent interruptions and/or distractions including the ability to prioritize and reprioritize tasks quickly
- Ability to maintain equanimity in the face of resistance, indifference, and hostility
- Ability to maintain confidentiality and to work with sensitive and confidential information using discretion and judgment
- Working knowledge of national, regional, and community resources
- Knowledge of self-care strategies and resources
- Knowledge base to provide education to patients, families, and other staff regarding clinical and patient care issues
- Ability to collect and relay information with accuracy and diplomacy, and effectively communicate verbally and in writing
- Ability to work with a multidisciplinary group, communicate with various providers and staff at different levels of patient care, and have excellent coordination skills. Excellent in planning, directing, and coordinating
- Ability to coordinate Program resources to coordinate multidisciplinary care across the continuum of care
- Skill in operating a PC and PC-based software applications, including Microsoft Office programs including Word, Excel, and PowerPoint
Knowledge of the Oncology patient population and practice operations
Preferred:
- Possession of Academy of Oncology Nurse & Patient Navigators certification
- Working knowledge of the UCLA EHR (Epic/CareConnect)
- Basic knowledge of insurance reimbursement systems
- Working knowledge of financial hardships and payer coverage
