They will serve as a liaison between the Operation Mend participants, Operation Mend staff and participant’s local case manager and care providers to ensure continuity of care. Using appropriate screening criteria, incumbent will work with the patient and accompanying family member(s) through intake process. The Operation Mend program serves veterans injured in combat or training while on active duty for any conflict designated by the United States Military. Many of these individuals have served in Operation Enduring Freedom (OEF), Operation Iraqi Freedom (OIF), and Operation New Dawn (OND) and are suffering from complex physical injuries, traumatic brain injury and/or post-traumatic stress.
Some evening/weekend appointments required.
Excellent writing and communication skills, and ability to effectively and positively interact in person, by telephone, e-mail, on web to veterans and their families in large group or small settings.
Ability to inspire, educate, and recruit military veterans and their families who need support with physical injuries, PTSD and TBI to come into the Operation Mend program for evaluation and treatment and ongoing care.
